The following items are required upon signing up:
1. PCA/HHA Certificate (Originals Only)

If the certificate was not issued this current year, a Certificate must be accompanied with your in-service certificates or you must be working with an agency within the current year.
2. Current Physical Form
(Please make sure that the Doctor’s signature, date, and stamp is on the form)
Must be recent (completed within six (6) months
a. Date PPD test was completed and read
(If the result is positive, we will need a copy of the Chest X-Ray Lab Report)
3. A copy of your lab tests for Rubella & Rubeola/Measles
4. Drug Screen & Results

Flu Vaccine is Now Required
In the event that you’re unable to be vaccinated, you must provide a letter from your Physician stating so. You will be required to wear a MASK if not Vaccinated.
7. One UNEXPIRED Picture ID: EITHER NYS ID, Driver’s License, Permanent Resident/Green Card, Work Authorization Card (If not a U.S. Citizen)
Or if born in the USA: US Passport, Birth Certificate
8. A copy of your Social Security Card (No Exceptions)
9. Two (2) Letters of Reference: MUST BE TYPED AND SIGNED
One issued by a training school, Or Previous Employer (work-related preferred), Or Professional (meaning from a Doctor, a Nurse, a Pastor, Teacher, Social Worker)
1. New York State License/ Certificate
(Please bring the original document with you for verification)
2. Liability/Malpractice Insurance
3. Approval Letter from NYS Department of Health
4. Current Physical Form
